This free mask form is like a blank canvas for your imagination…
Just print it out and stick it to cereal box cardboard. You’ll need one carton for a face mask, or two cartons for a helmet-style mask.
Once you have the pieces cut out and taped together, you can go wild with the details. Need a Tasmanian tiger mask, maybe? Or a space alien, or a deep sea creature?
Whatever you can dream up, you can make it, using this pattern for your base. Just fill out this form and I’ll send you the download link:

This mask form is really simple, but you can use it to make any mask you can dream up. (I made this owl mask with mine.)

Are you not seeing the green button that lets you order the pattern? Some ad blockers mistake email forms as ads, but there are no ads on my site. If you’re using an ad blocker, try turning it off temporarily, and then refresh the page. If that doesn’t work, have you tried using a different device?

Good afternoon, I’m also having trouble finding the link for the form, can you help me?
Hi Beth. There should be a green button under a text box that asks for your email address. Is it not showing up? It should be right below the third paragraph. Most people can see it, and it’s been tested on a lot of different browsers, but if it isn’t there you might have a security setting that’s hiding it. If it isn’t showing, contact me and I’ll make sure you get the pattern.

I have tried twice to get this mask pattern

Hi Rob. The form for requesting the download is below the video. Is it not showing up on your browser? I’ve tried it in two different browser and my android phone, and it’s showing up. I tried to change the color of the button, but my system won’t let me for some reason.
I just figured out how to make the button easier to find. Now it’s green. Does that help?
Got your email with a pdf, but only instructions and no template.
Hi Judy. Did you click on the link in your email? And when you opened the PDF, how many pages were there? There should have been 10 pages, with the pattern on pages 3 – 6. Is that not what you received? I tested it, and it appeared to be working, but you might need to download the file again. Give it a try, and let me know if it works this time.
